Return Material
If you need to return your product for repair the Return Material Request form must be completed prior to sending the equipment MTI. To return the equipment complete the form below and customer service will provide you with an RMA number. This number must accompany the return material. Please be sure to package your equipment properly to avoid further damage. MTI strives to repair your product quickly, turning it around within three days. Unless prior arrangments are made we will ship your return by Fedex Ground day shipping.

Notice:
- All parts shipped back to MTI must be shipped back in adequate packing so as to ensure that no further damage is caused during shipping. If possible, the original packaging provided for shipment of the product should be used. Otherwise, please contact MTI for information on the best methods for shipping the product. The following are some general rules for shipping product:
- Use large bubble wrap or foam to pack the product for shipment. At least 4 inches of packing should be supplied around all of the product
- All products should be shipped in a completely contained and enclosed box.
- MTI ships all repaired material via FedEx Ground, unless otherwise stipulated. The Ground shipment is free to the customer, but any expedited shipping will be charged to the customer.
- It is the sole privilege of MTI to determine the extent to which warranty covers a product. Please refer to the MTI Warranty Policy for more information.
- Parts and labor provided in the repair of a product is warrantied for 90 days, starting at the date of shipment.
- No on-site labor is provided for the repair of product.
- MTI does not repair any product that has been in use for 10 years or more. MTI does not repair computers that are 5 years old or older.
- All materials shipped from MTI to the customer or F.O.B. our docks. It is the responsibility of the customer to inspect and accept or reject product that is damaged due to shipping. MTI does not cover any damages of product due to shipping.
- The RMA number must be clearly marked on the outside of the return material package. Packages that are not clearly marked with the RMA number will be rejected by the MTI shipping/receiving department.
